Orforglipron

Foundayo · LY3502970 · not a peptide (small-molecule GLP-1 pill)

Lilly's once-daily GLP-1 pill. It is not a peptide: it is a small molecule that switches on the same receptor as semaglutide. Approved by the FDA as Foundayo in April 2026 and by the UK in August 2026.

What it actually is

Orforglipron is a small-molecule drug, not a peptide. It activates the GLP-1 receptor, the same target as semaglutide and liraglutide, but it is an ordinary chemical rather than a chain of amino acids, so it works as a daily pill taken with or without food.

The FDA approved it as Foundayo on 1 April 2026 for chronic weight management, and the UK's MHRA approved it in August 2026 for weight management and type 2 diabetes. It is a prescription medicine.

Carl’s one-liner: A GLP-1 you swallow, with no fasting rules. It is listed here because people sell it next to peptides; it is not one.

How it’s thought to work

What the research says

Strong. ATTAIN-1 (phase 3, 3,127 adults without diabetes, 72 weeks) found weight change of -7.5%, -8.4% and -11.2% on the three trial doses, against -2.1% on placebo, counting everyone who started. For context, semaglutide 2.4 mg managed -14.9% in its own 68-week trial (STEP 1); different trials, so not a direct comparison. In type 2 diabetes orforglipron beat oral semaglutide on HbA1c head to head (ACHIEVE-3), with more stomach side effects. ATTAIN-MAINTAIN showed it held most of the weight people had lost on tirzepatide or semaglutide after switching.

Reported protocols

These are the doses the peptide community actually reports running (Reddit & forums), alongside published research — logged for education, not as a recommendation to use.

ParameterCommonly reported range
Label dose0.8 mg once daily for at least 30 days, then 2.5 mg, then 5.5 mg; may go to 9, 14.5 or 17.2 mg (maximum), at least 30 days per step. Prescription medicine.
Trial vs label numbersThe trials used an investigational formulation at 6, 12 and 36 mg. The label reports the same trial as Foundayo 5.5, 9 and 17.2 mg. The numbers are not interchangeable.
FrequencyOnce daily, any time, with or without food
RouteOral tablet, swallowed whole
InteractionsMaximum 9 mg with strong CYP3A4 inhibitors; pill contraception needs a backup method for 30 days after starting and after each step up
Grey-market noteAnything sold as research orforglipron powder or capsules has no reliable link to these doses. The approved tablet is the version with a known dose.
Reality check: Orforglipron is an approved prescription medicine in the US (April 2026) and UK (August 2026), with the full GLP-1 label: a boxed thyroid C-cell tumour warning carried over from the class (the label notes orforglipron itself is not active in rats or mice and caused no tumours there), plus pancreatitis, gallbladder disease and pregnancy warnings. It is not a peptide, so SAHPRA's peptide list does not name it, but the Medicines Act still requires registration before any medicine is sold in South Africa, and SAHPRA warns against GLP-1 products bought from online sellers, gyms and social media.

Common use cases

GoalCommunity protocol notes
Chronic weight managementthe FDA-approved use, alongside diet and activity
Type 2 diabetesapproved for this in the UK; beat oral semaglutide on HbA1c
Keeping weight off after an injectableATTAIN-MAINTAIN switched people from tirzepatide or semaglutide

Mixing it (reconstitution)

Freeze-dried peptides must be reconstituted with bacteriostatic water before they can be measured. Carl’s calculator turns “mg in the vial + ml of water + target dose” into “units on the syringe” — and tells you how long the vial lasts.

Injection & handling

No injection. Foundayo is a tablet taken once a day, at any time, with or without food, swallowed whole (not split, crushed or chewed), and never more than one a day. Each dose step lasts at least 30 days. If you rely on the pill for contraception, the label says to switch or add a barrier method for 30 days after starting and after each increase.

Carl’s quick FAQ

Is orforglipron a peptide?
No. It is a small-molecule drug that activates the GLP-1 receptor. It is listed here because it gets sold alongside peptides.
Orforglipron vs oral semaglutide?
Both are daily GLP-1 pills. Orforglipron was designed without food or water restrictions and beat oral semaglutide on blood sugar in ACHIEVE-3, but with more stomach side effects and more people stopping.
Why are the label doses different from the trials?
The trials used an investigational formulation (6, 12 and 36 mg). The approved tablet is a different formulation, and the label reports the equivalent Foundayo doses (5.5, 9 and 17.2 mg).
How much weight does it take off?
About 11% at the top dose over 72 weeks in ATTAIN-1, counting everyone who started. Below semaglutide 2.4 mg in its own trial (about 15% in STEP 1).
The honest summary: Orforglipron is a real, approved medicine whose selling point is convenience, not record weight loss. It is not a peptide, and the only sensible way to take it is on prescription; a grey-market version throws away its one big advantage, a known, quality-controlled dose.
